- IPK minimum 3.0
- Bachelor’s and Master’s degree certificates and transcripts accredited B. For prospective students with non-health D4/S1 backgrounds and non-public health masters, matriculation is required.
- Permission letter from work institution/agencies to continue doctoral studies, for those already working (format has been provided)
- Statement letter of ability to complete the study (format is provided)
- Certificate of guarantee for payment of study fees (stamped 10000) (format already provided)
Requirements Doctor by Research :
- Have research experience in the health sector, as indicated by scientific papers that have been owned or articles that have been published in scientific journals;
- For practitioners, can show the results of managerial work in the health field;
- Proficient in English (TOEFL 500) upon registration;
- Obtain recommendations from one former supervisor/lecturer during S2 studies and one supervisor (for those who are already working and are bound by their work). For those who have not worked, get a recommendation from 2 former supervisors/lecturers during their master’s degree;
- Include a research plan for dissertation / pre-proposal research;
- 2 pieces of 4×6 photos.
